Social Services Assistant
The primary goal of Housing & Urban Development/VA Supported Housing (HUD-VASH) is to move Veterans and their families out of chronic homelessness. The primary component of the program is VA case management services, designed to improve Veteran's health and mental health, enhancing their ability to remain stable, housed and community-integrated.
The Social Service Assistant is part of the HUD-VASH team, a program within the Healthcare for Homeless Veterans Program (HCHV). The Housing Specialist works specifically with the HUD-VASH PROGRAM providing chronically homeless veterans with housing referrals, and transports veterans to and from housing viewings, and any other assistance related to securing section 8 housing.
The incumbent serves as the Social Service Assistant for the HUD-VASH Program for the Healthcare for Homeless Veterans program located at the VA Northern California Health Care System (VA NCHCS). The HUD-VASH team is a key component in VAs continuum of homeless services, representing VAs only means of providing permanent housing to homeless Veterans and service a critical role in VAs five Year Plan to End Homelessness among Veterans.
